PIVKA-II assay is indicated for use as an aid for diagnosis of HCC (hepato-cellular carcinoma), monitoring of high risk patients (hepatitis C virus infections, hepatitis/cirrhosis, hepatitis
B virus infections) for development of HCC, and in management of HCC.
PIVKA-II is abnormal des-carboxylated prothrombin (DCP) present in vitamin K deficiency or in patients treated with warfarin or phenprocoumon. The des-carboxylated prothrombins are functionally defective
because they cannot bind calcium and phospholipid. Measuring PIVKA-II permits the detection of biochemical Vitamin K deficiency before the results of conventional coagulation tests are changed or
hemorrhage occurs. PIVKA-II is absent in normal individuals, but in liver disease and liver malignancy PIVKA-IIs can be present even without Vitamin K deficiency.
